The scan is clean. The blood work is normal. The patient is still in pain, and the next decision is yours.
Headache Medicine is a single-author clinical textbook written for the neurologist, headache specialist, trainee and primary care physician who has to reach a diagnosis in twenty minutes and defend a treatment plan afterwards. It covers the primary headache disorders in the depth a treating clinician needs: migraine in all its forms, tension-type headache, cluster headache and the other trigeminal autonomic cephalalgias, medication-overuse headache, and the rarer primary headaches, together with the secondary headaches that arrive in the same clinic with the same normal scan: vascular, intracranial pressure, infectious, inflammatory, post-traumatic, substance-related, structural and neuropathic causes. Separate chapters address children, older adults, pregnancy, the emergency department and the organisation of headache care.
The book follows the third edition of the International Classification of Headache Disorders throughout. Every treatment table carries an evidence grade. Every chapter is built around tables, decision pathways and original schematic figures, opens with key points and closes with a composite case from the consulting room. The therapeutic chapters reflect the treatments that have arrived since the first CGRP antibodies, including the 2026 AAN/AHS practice guideline, and they are candid about what those treatments still cannot do. An afterword sets out ten arguments the field has not settled and where the author stands on each.
Appendices gather the ICHD-3 criteria in quick-reference form, a formulary of every drug in the book, the instruments a headache clinic uses, a glossary and one-line summaries of the major trials. References are collected by chapter at the end of the book.
Written from twenty-five years of neurology practice in which the author has seen more than one hundred thousand new headache patients, this is a complete, single-voice reference for headache medicine as it is practised today. Hardcover, 820 pages, 57 original colour figures, 338 tables.
